Remuneration: $40.02 to $40.52 per hour + 12.5% superannuation + salary packaging + remote allowances

The Position
The Carer Support Planner provides carer focused services and consults with carers to identify their goals and ensure an appropriate support plan is prepared, implemented, and evaluated. You will need to have the ability to work one evening shift per week (11:30am to 7:36pm) and be comfortable working autonomously.
Responsibilities of the role include (not limited to):
Contribute to the intake, planning, and support function of carers
Develop partnerships with local agencies to establish ongoing promotional activity and warm referral processes for carers interacting with those organisations
Deliver a Carer focused service which consults with carers and involves them in service planning, delivery and evaluation to assist in continuous improvement
Establish local carer peer support groups, including training volunteers
About You
This position undertakes a combination of duties including administration, budgeting, contractor engagement, and some scheduling; so being adaptive to change and flexible in how work is undertaken, along with excellent communication skills, are prerequisites.
Minimum Qualifications:
National Police Clearance (valid within 6 months), Proof of Australian Working Rights, Working With Children Check, Current Drivers License, and First Aid Certificate
Although not an essential requirement, a relevant qualification will be highly regarded, with candidates possibly having an educational background in social work, counselling, community or healthcare services.
