We are seeking an experienced and dedicated Senior Physiotherapist to join our multidisciplinary team within the Central Coast Transitional Aged Care Program (TACP). This position plays a crucial role in providing high-quality, client-centered physiotherapy services to older individuals transitioning from hospital care back to their homes, enabling them to maintain their independence and function.
In this leadership role, you will guide and supervise a team of junior physiotherapists and allied health assistants while working collaboratively with other health professionals to deliver an integrated, therapy-focused service. As part of the TACP team, you will help ensure that older people receive the appropriate care and support to avoid premature admission to residential aged care.
An Eligibility List (E-List) or talent pool may be created from this recruitment episode to assist in filling future permanent, temporary, or casual full-time and part-time positions.
Key Responsibilities:
* Provide expert physiotherapy services to clients within the community and residential Transitional Care Units.
* Lead and mentor a team of Level 1-2 physiotherapists and allied health assistants.
* Work closely with senior therapists and other healthcare professionals to ensure coordinated service delivery across multiple geographic sites.
* Deliver evidence-based, client-focused care to optimise the functional independence of older people following a hospital stay.
* Contribute to continuous quality improvement efforts and maintain high standards of service delivery.
* Foster a collaborative, positive, and supportive team environment.
About You:
* You have a current registration to practice as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA).
* You hold a current valid Australian driver's licence and the ability and willingness to travel across sites for meetings and clinical care provision.
* You have the ability to meet supervision needs of students and/or less experienced staff.
* You have the ability to lead planning and evaluation of team activities.
