Grampians Health is seeking a motivated hospital pharmacist with established clinical and leadership skills to work collaboratively with the Lead Pharmacist - Medication Safety to co-ordinate the medication safety program across Grampians Health, a large and diverse multi-campus regional health service.
The position is for a fixed term, replacing the current Senior Pharmacist - Medication Safety, who is being seconded to another position. The successful applicant can choose any Grampians Health campus as their primary work location, but the role requires a whole of Grampians Health focus.
Key Responsibilities:
* Develop and implement system-based, consumer-focused medication safety quality improvement strategies to reduce the risk of harm to patients and enhance medication safety
* Monitor and evaluate medication safety systems through audit and incident report investigation, analysis and reporting
* Provide medication safety related training and education to clinicians and consumers, as required
Skills and experience:
* Currently hold general registration as a pharmacist with the Australian Health Practitioner Regulation Agency (AHPRA)
* At least 5 years hospital pharmacy experience, including broad exposure to the different aspects of the medication management pathway
* Demonstrated ability to effectively engage stakeholders to achieve desired outcomes
* Demonstrated ability to effectively plan, organise, prioritise and manage time with ability to deliver outcomes to agreed timelines
