About Aged and Rehabilitation Division
The Monash at Home - Aged and Rehabilitation Care service launched in December 2020 and is a fundamental part of the broader Aged and Rehabilitation Division. Monash at Home - Aged and Rehabilitation Care provides safe and high-quality home-based subacute level care for individuals with complex medical, functional, and often cognitive issues who require comprehensive, medically led multidisciplinary assessment and optimisation. Monash at Home - Aged and Rehabilitation is seen as an alternative to hospital based subacute care, the service is designed to facilitate an earlier discharge from acute care and/or avoid subacute hospital stay entirely. Patients within the service receive at least daily visits from a member of the multidisciplinary team and access to 24-hour telephone support. Patients may also access brokered services to assist with Activities Daily Living (ADLs) where there is no other support available.
About the Role
Consultant Rehabilitation Physician who will provide clinical services to the Monash at Home service. This service provides a bed substitution service for patients residing in our catchment. You will work with a multidisciplinary team comprising of a Consultant Geriatrician's, Advanced Trainee's in geriatric and rehabilitation medicine, HMO's, nursing and allied health clinicians including; physiotherapists, occupational therapists, dieteticians, social workers, speech pathologists and neuropsychologists. You will be responsible for providing patient centred, evidence based care to patients in their own home both face to face and via TeleHealth.
* Provide clinical leadership through active involvement in patient care
* Be available to medical colleagues for consultation and liaison as appropriate regarding patient care
* Contribute to the professional and corporate life in Victoria's largest public health service by participation in appropriate committees and educational platforms.
* Contribute to ongoing service improvement and clinical and workforce innovation to ensure Monash Health provides high quality accessible Rehabilitation Medicine services within allocated resources and meets the needs of Monash Health patients.
