About Your Role
An exciting opportunity for a highly motivated Registered Nurse with appropriate knowledge and skills to join the Integrated Ambulatory Care team at Wagga Wagga Health Service.
The Integrated Ambulatory Care Services consist of a community faced Hospital In The Home (HITH) service, Day Infusion Medical Unit, minor medical procedure/infusion and investigation unit (PIXI) and Rapid Assessment Clinic. In this role you will be working across all these units to promote a community faced inpatient and outpatient model of care.
The successful applicant will work within our service comprising of a cohesive team of registered nurses and a multidisciplinary team lead by the Director of Integrated Care Services and Nurse Unit Manager of Ambulatory Care Services. This position is responsible for; focused assessment and management of patients with complex conditions, ensures patient centered care with high level coordination and communication between the inpatient and home based settings, advanced clinical assessment using diagnostic capability, planning and engaging others.
*Nurses with a background in emergency nursing, aged care, primary health or chronic disease, experience with CVAD, wound care would be favourable.
