Your expertise will be invaluable in assessing patients' needs, developing tailored intervention plans, and providing comprehensive therapeutic interventions. In these roles, you will collaborate with a multidisciplinary team of healthcare professionals, fostering a compassionate and patient-centred approach to care. You will liaise with other health professionals, service providers, and the community to facilitate the safe and timely discharge, improved independence, and occupational outcomes for patients.
