Qualifications: Current AHPRA registration About the position: Applications are invited from Registered Nurses who would like to take up the opportunity to join our dynamic Community Health team to work in District Nursing team in Yarrawonga. The primary role of the District nurse is to provide a person centred approach focusing on maximising functional capacity, home based care, improved self-management strategies and coordination of service delivery to clients in their own homes. Successful applicants will be supported with an internal orientation and training and will be supported throughout by the Clinical Coordinator of Community Nursing, District Nursing colleagues and the Operational Director of Community Services. About you: You must be committed to providing evidenced based practice, demonstrate an understanding of and commitment to nursing in the community setting and quality improvement. Applicants should have the appropriate skills including the ability to plan care, assess clients, and liaise with other care providers. A current driver’s license is essential, and post graduate qualifications are highly regarded KEY SELECTION CRITERIA: Essential: Registered Nurse or Registered Nurse/Midwife Minimum of 3 years’ experience in providing direct clinical care and experience in wound care. Experience in working with vulnerable and aging clients. Desired: Experience with PICC/Port access and care. Demonstrated leadership ability with a clear understanding of the responsibilities of being a District Nurse Demonstrated ability to work collaboratively and consultatively as part of a multidisciplinary team Demonstrated ability to work autonomously as a lone worker in the community setting Ability to communicate effectively and appropriately with all major stakeholders both internal and external Ability to problem solve and make decisions in a variety of complex situations Demonstrated knowledge of professional standards of practice, legal and ethical requirements and industrial relations Demonstrated ability to use Information Communications Technology For full details of position, please refer to the attached Position Description.
