Role Summary:
We are seeking a dedicated clinician to join our Youth Mental Health team. As a key member of our multidisciplinary team, you will provide evidence-based treatment and support to young people aged 12-25 years in the Barwon Region who are experiencing mental health issues related to drug and alcohol use or homelessness.
About the Role
The successful candidate will be responsible for delivering best practice care to complex clients with multiple needs. This will involve working collaboratively with other healthcare professionals, families and carers to develop and implement individualized care plans that promote recovery and well-being.
Key Responsibilities:
* Provide assessment, diagnosis and treatment services to young people with mental health disorders, substance abuse issues and/or homelessnes
* Develop and implement individualized care plans in collaboration with young people, their families and carers
* Contribute to a multidisciplinary team approach to ensure comprehensive care is provided
* Maintain accurate and timely records of client interactions and progress
