Care Coordinator (Community) - Aged & Disability Right at Home is a leading provider of quality in-home care.Our mission is to improve the quality of life for those we serve ensuring the Right Care, Right at Home.We support people living with complex and post-operative care needs, dementia and cognitive decline, seniors, and adults living with a disability including NDIS participants.Our services help clients to remain safe and independent in their homes.We are a fast-growing, family-owned business, seeking a highly positive team player to join our Greater Logan office based in Shailer Park.We are seeking an experienced, enthusiastic and highly motivated Care Coordinator to coordinate, manage and administer the delivery of care services to our clients.Our clients are a mix of Home Care Package, NDIS and Private Pay across all age groups.This role is responsible for: Managing and coordinating services for home care package clients Delivering outcomes/goals for our clients based on their care/NDIS plan Monitoring HCP budgets for clients depending on level of Government assistance Working collaboratively with our clients to coordinate supports and services to improve their quality of life Conducting client care assessments and care planning with a person-centred care approach in alignment with goals/outcomes and within budget restraints Understanding, managing and working within government contract requirements Supervision and management of Care Staff Scheduling appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements with our unique carer matching at the forefront of decisions Coordinating the provision of quality person-centred care according to the client's needs, service/care plans and available funding in accordance with budget Building relationships with local stakeholders Maintaining a positive attitude with the ability to work in an ever-changing environment Exhibiting high emotional intelligence Participating in a rotating roster of weekday and weekend 'on call' with other staff members Qualifications Either:Cert III/Cert IV (Aged/Disability Care) with a minimum of 1 year of experience in a coordination role OR Registered Nurse (RN) in community nursing with 1-2 years of current AHPRA registration OR Enrolled Nurse (EN) in community nursing with 1-2 years of current AHPRA registration Experience within the aged care/disability/mental health in a community setting Understanding of Aged care & Disability standards and consumer-directed care First Aid, current CPR and National police check Working with children Blue Card (or willingness to obtain) Driver's licence - Australia Reliable vehicle High level of computer literacy and understanding of technology Understanding of clinical governance and continuous improvement Possessing a 'can-do' positive attitude and ability to work well in a team environment Preferred Skills Previous experience with rostering is highly desirable Experience with Microsoft Word and Excel An outgoing personality with high energy levels, good communication skills and personable telephone manner Ability to problem-solve and think quickly under pressure Ability to multitask with a strong attention to detail while keeping calm and focused Excellent networking and consultation skills to develop new referral sources and relationships with appropriate external organisations Excellent customer service skills with exceptional verbal and written communication skills Sound knowledge and understanding of home care services and guidelines #J-18808-Ljbffr
