NDIS Support Coordinator & Recovery Coach
Reach Social Supports was founded in 2020 to address a growing absence in the disability sector of high-quality community focused NDIS supports in the Gippsland region. Reach now provides social group support, one-on-one support, support coordination and more to the NDIS communities of Gippsland, and Melbourne. Our services are focused on the client: assisting them to achieve their personal goals and to be a part of an inclusive community of like-minded individuals who are there for each other.
The Role - Support Coordinator
The Support Coordinator's role is to facilitate growth in client capacity and ability to manage and engage with their NDIS services. Support coordinators support and empower their clients through knowledge, expertise, connection to their community, and assist them to make the most of their NDIS plan. The role will report to the Recovery Coordination Manager.
This role is suited to someone who has a high degree of initiative and the ability to deal with a broad range of tasks. Reach also recognises the tendency for some people to only apply to roles where they meet 100% of the requirements. Experience in the NDIS as a support coordinator, or a solid understanding of how the NDIS functions is highly desirable but not required.

Key responsibilities for the Support Coordinator:
* Provide Support Coordination services under NDIS Quality & Safeguards Commission guidelines and Reach Policy and Practice.
* Build strong and effective relationships with external provider organisations.
* Monitor NDIS plan budgets and support services to ensure plan budgets are utilised effectively and services are working towards the NDIS participant to achieve their plan goals.
* Liase with participants and all relevant stakeholders.
* Promote a positive and professional image of Reach Social Supports.
* Achieve set targets, KPI's and caseload in a timely and effective manner.
To be successful in this role you will:
* Possess an understanding of mainstream, community, formal and provider support available to NDIS participants in the region.
* Demonstrate the ability to interpret and effectively communicate the technical language used in a participant's NDIS plan.
* Demonstrate you can work collaboratively with participants, their families, carers, and other services to design, plan and implement a recovery plan, and assist with the coordination of NDIS and other supports.
* Be open to learning on the job and highly empathetic.
* Demonstrate ability to work independently in an agile environment, be flexible, resourceful and adaptive.
* Demonstrate effectiveness in the delivery of people-centred services.
* Demonstrate organisational and problem solving skills, with an ability to manage competing priorities.
* Demonstrate excellent interpersonal, teamwork, communication skills, an eye for detail and the ability to problem solve.
* Demonstrate high-level computer literacy including the use of e-mail, word processing, spreadsheet, and database applications and the capacity to learn new programs.
* Experience as a Support Coordinator will be highly looked upon, or an understanding of how the NDIS functions.
* Experience supporting high needs clients is highly desirable.
* Qualifications (Cert IV or greater in Community Services, Mental Health, or Disability) and/or experience of 2 years or more supporting people in mental health, while not required, will be highly looked upon.
Must hold or be willing to obtain:
* Valid Driver's License with no restrictions.
* Current First Aid Certificate (or willing to obtain).
* NDIS Worker Screening Check.
* NDIS Worker Orientation Module.
* Current Police Check.
