Carer Support Clinician, Community Health
Temporary Part Time Opportunity: Provide supportive care to carers and their families in a community health setting.
Job Details:
* Hours: 64 hours per fortnight
* Duration: Temporary part-time position until further notice
About the Role:
This role involves working within a multi-disciplinary team environment to provide government-funded carer support services. The successful candidate will be responsible for undertaking holistic assessments of clients' needs, developing goal-directed care plans, responding to emergency and practical needs of carers, and coordinating service delivery.
Key Responsibilities:
* Undertake assessments and develop care plans for clients
* Provide support and coaching to carers to help them achieve their goals
* Respond to emergencies and practical needs of carers
* Coordinate service delivery and refer to other service types as required
