To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region.

About you: We are looking for an experienced Client Intake Coordinator to play a vital role in supporting participants and ensuring high-quality service delivery.
Qualifications Qualification: Minimum Certificate III in Disability, Mental Health, Community Services (Diploma/Bachelor's preferred) WWC & NDIS Worker Screening AU Driver's License Police Check 100 Points ID Selection Criteria 2+ years in disability services, intake coordination, or a similar role Strong communication, stakeholder engagement, and data management skills Your tasks may include but not be limited to the following Engaging with new participants, conducting intake assessments, and coordinating services Managing referrals and onboarding participants for accommodation, community access, and support services Building strong relationships with participants, families, and stakeholders Ensuring compliance with NDIS regulations and organisational policies Promoting Care & Connects through networking events and inter-agency meetings Any administrative & reporting tasks as and when required by Management How to Apply?
