Together we can do more.The Discharge Patient Flow Concierge (DPFC) will be responsible for providing direct administrative and clerical support to Allied Health clinicians (in person, phone, email and teams) to help ensure timely coordination of documentation, communication and logistical arrangements to facilitate efficient patient discharges.
