capacity, you will use your empathetic, caring and community-focused nature to undertake comprehensive assessment and screening of cancer patients for psychosocial and supportive care needs, facilitate the delivery of cancer care consistent with evidence-based guidelines and ensure prompt referral to specialist, allied health and supportive care services.You will work proactively and confidently both independently as well as alongside the multi-disciplinary team.Key Responsibilities: Provide effective leadership and management to the multidisciplinary clinical care services team, including contractors, to meet client needs and service delivery priorities.Facilitate and provide client care consistent with COUCH's Living Well Survivorship Client Pathway to maximise client health and wellbeing outcomes.Contribute to a high-performance culture across the clinical services team by implementing clear expectations aligned to organisation and service goals and priorities, ensuring each team member is accountable for their success.Manage, implement and maintain client and clinical databases eg.Gensolve to maximise clinical system data capture and integration which supports quality care planning, delivery, evaluation and reporting.Manage, monitor and evaluate clinical quality, risks including WHS standards and compliance.Skills and Experience: Division 1 Registered Nurse with current AHPRA Registration (Nursing & Midwifery Board).Minimum 5 years' post registration experience as a Registered Nurse.Ideally 1 year of oncology, palliative or mental health experience.Ability to lead, support, communicate and collaborate in a multidisciplinary team environment.National Police Check.2 recent professional referees.Exceptional communicator with empathy, be extremely effective independently.Essential Criteria Demonstrated evidence based clinical skills in assessment and care planning either in a nursing setting or similar.Demonstrated experience in implementation of digitalised systems and processes including roll out of evidence-based programs and activities.Ability to provide effective leadership and multi-disciplinary teamwork skills in relation to the management of a small team including contractors.Ability to develop, manage and monitor performance outcomes for evidence-based programs and activities.Ability to provide formal and
