Currently we are seeking Coordinator Complex Care Pharmacists for our CoNeCT and CoNeCT Mental Health Expansion (MHE) service.
This is a recruitment pool for casual, fixed term full time and fixed term part time appointments, with the possibility of extension(s).
We current have the following vacancies are available to be filled:
1. 1 x fixed term part time CoNeCT Pharmacy position for 6 months (commencing early December 2023). Initially based at South Guildford but may require relocation to another North Metropolitan base.
2. 1 x fixed term full time CoNeCT MHE Pharmacy position 2 years and 3 months (commencing February 2024 and ending 30 June 2026). Mainly based in Myaree.
The Complex Needs Coordination Team (CoNeCT) operates across the metropolitan area under the South Metropolitan Population Health Unit, with small teams located across the Perth metropolitan area.
Patients who are identified by hospital staff as being at risk of further hospitalisation are supported by CoNeCT Care Coordinators to manage their healthcare at home through chronic condition self-management support, linkage to appropriate services – including mental health supports and coordination of complex care needs. You will be working closely with this team in addition to liaising with hospital clinicians, GPs and community health providers. You will use your expertise in medication management to reduce the risk of medication-related harm and improve health outcomes for this vulnerable cohort of patients.
In this role, you will be responsible for the following:
Clinical: To provide an advanced level of hospital and community focused care coordination to an identified “at risk” client target group of people and their carer, whose care and support needs are assessed as complex. To promote safe, rational and cost-effective drug therapy by providing clinical pharmacy services which include monitoring of the patient’s medicine regimen, providing drug information and undertaking teaching and research.
Education, Liaison and Networking: To network and link with key stakeholders across the care continuum of acute care, sub-acute care and community settings. This will include education and liaison to actively build relationships across services working with the client group. Supervises CoNeCT staff, coordinates duties and manages performance.
Evaluation and Reporting: To actively engage in development and implementing of an evaluation process, including recording, collecting and reporting key performance information. Conducts research activities in line with the Complex Care Coordination program development and future planning to ensure evidence-based practice healthcare.
