In-home clinical assessments, reviews and care needs for clients in the Bendigo areaProvide clinical advice and support to clients and provide referrals to services where more specialized support/advice may be requiredProvide collaborative leadership, support and supervision to Care Advisors and Home Care staffWork closely with local allied health professionals to facilitate responsive care in our clients' homesProvide health education and promotion, including social support groups and carer educationPromote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management and achieving clinical support goalsPermanent part-time opportunityWeek 1: Wednesday to Friday 9am-5pmWeek 2: Monday and Tuesday 9am to 5pmMixture of visiting clients in their homes and also working in the officeAbout you: This is the place for you if you have:
