About the Role
1. Provide a high-quality specialist Social Work service to consumers and carers of the Upper Hunter Palliative Care service to ensure the provision of optimal healthcare outcomes for consumers of the service.
2. Provide patient centred clinical advice to clients, carers and other health care professionals to ensure delivery of innovative clinical practice models and therapeutic techniques
3. Ensure client centred care is delivered within professional, organisational, legal and ethical boundaries and reflects evidence based, best practice knowledge and practice
4. Liaise with other care and service providers to ensure seamless continuum of care from hospital to community that meets the needs of patients and carers
5. Participate in continuous improvement initiatives to enhance service delivery and patient/ client outcomes
6. Mentor and supervise junior professionals to ensure their ongoing professional growth and enhanced application of clinical knowledge.
7. Work within risk management, safety, infection control and quality frameworks applicable to the organisation

Where you'll be working
12. The successful applicant will be based at Muswellbrook Community Health as part of the Upper Hunter Social Work team and will work closely with the Upper Hunter Palliative Care service.

13. The successful applicant will be supported by the Upper Hunter Social Work team which comprises of a small team of Social Workers who are located together at “the Cottage” on the Muswellbrook Health service site.
14. You will be line managed by the Social Work Head of Discipline who is also based at the Muswellbrook Health service campus and will have access to regular monthly clinical supervision provided by this clinician.
15. You will also have access to relevant training, education and support relevant to their role and will be well supported by the Social Work team, the Upper Hunter Palliative Care team and the wider Muswellbrook community health team.
