EIPSRS provides an individualised, tiered program of recovery support of up to 12 months delivered to clients with the level of engagement determined by their needs. Services include individual support, one-on-one outreach services, and group work. The services are designed to enhance recovery, achieve wellbeing goals, build capacity and life skills, maintain sustainable housing, improve relationships with families and carers, develop social and community connections, and live safely and independently in the community.We are seeking a compassionate, self-motivated Community Mental Health Practitioner. Working from home, you will deliver assertive person-centred outreach services to clients to achieve their recovery goals, build capacity and living skills, strengthen social and community networks, maintain housing, and live independently. As this position requires travel across the Melbourne metropolitan area, a fully maintained company car is provided.Key ResponsibilitiesWork with clients to enable them to develop independent living skills and a meaningful life by supporting them through a strength-focused, staged approach to recovery using agreed practice techniques and approaches.Plan and develop group work programs that will assist clients to build their skills, focus on their recovery, and work towards transitioning back to their natural community.Engage with clients to fully understand their need for assistance from local service providers with clinical mental health, physical health, education and employment, eligible entitlements and benefits, housing, transport, recreation, and social connections.Facilitate, as appropriate, the re-engagement and maintenance of family and carer relationships.Ensure all clients have an active recovery, treatment, or case coordination plan consistent with the EIPSRS Operational guidelines.What You’ll BringTertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.Expertise in working with people with mental health issues, complex needs, and interacting with families and carers.Proven experience in Community Services, Mental Health, Disability, Social Welfare, Housing, Healthcare, or Government sectors is required.Demonstrated knowledge and experience in the application of theoretical approaches, practices, and appropriate service responses including family inclusive practice, trauma informed theory, cognitive behavioral therapy, motivational interviewing, and harm reduction therapy.Ability to co-design, co-produce, and co-facilitate groups and education support.BenefitsSalary packaging up to $15,899 p.a. allows you to allocate a portion of your pre-tax income to cover living expenses (e.g. rent, mortgage, child care, car leasing expenses etc.)Learning, development and career opportunities.Fitness Passport - Access to our corporate health and fitness program, providing you and family with access to a wide choice of fitness facilities.Fully maintained company car for private use.Interested?To obtain a position description and apply online visit Mind Careers - reference number 496993.
