The Access Aged Care team is currently offering co-managed primary care services to residents in Aged Care facilities throughout Australia.
Your New Role
In this role, you will provide advanced nursing assessment, diagnosis, care, and intervention to Aged Care Residents within the scope of a Nurse Practitioner. This position involves on-site work with the possibility of some telehealth responsibilities. The Nurse Practitioner delivers high-level clinical nursing, leadership, management, and governance to patients. Collaborating with the wider medical team, the Nurse Practitioner assesses, diagnoses, treats, and manages patients' health concerns, emphasizing preventative health care. As a clinical expert, the Nurse Practitioner provides leadership within their scope of practice.
This role is onsite working 2 days per week with occasional telehealth and will be situated at facilities in and around the Sunshine Coast.

Qualifications
* Bachelor's Degree in Nursing
* AHPRA registration as a Registered Nurse with endorsement as a Nurse Practitioner.
Essential Criteria
* Advanced clinical experience in assessment, treatment, and health promotion within the scope of practice
* Demonstrated extensive professional and/or leadership experience
* Clinical problem-solving skills, including conceptual and analytical ability
* Advanced interpersonal, communication, and negotiation skills
* Knowledge and experience in quality improvement and evidence-based practice
* Familiarity with relevant legislation, health regulations, and obligations
* Computer literacy
* Evidence of appropriate and continuing professional indemnity insurance
* Current Driver's Licence and reliable vehicle
