About the role
Transition care services support older people who have been discharged from hospital or a subacute facility to undertake a restorative time limited low intensive therapy program to help improve general function and overall independence and to make an informed choice about future accommodation needs.
The program provides short term support and active management of older people at the interface of the acute/subacute and residential aged care sectors. It is goal orientated, time limited and targets older persons at the conclusion of a hospital episode who require more time and support in a community environment to complete their restorative process. The objective is to optimise their functional capacity and finalise and access their longer-term care arrangements.

This successful applicant will carry out the following key accountabilities in accordance with the Metro North Health's values:
Clinical Practice
1. Deliver quality social work clinical services across a variety of caseloads in a Community Transition Care home-visiting model in accordance with prescribed professional and ethical standards. Social Work clinical service delivery is aligned with a restorative focus in a Community Transition Care Environment.
2. Undertake Social Work routine duties independently, with more complex clinical decisions and problem solving made under the clinical practice supervision or professional guidance of a more experienced Social Worker
Communication/team participation
3. Apply communication, consultative and interpersonal skills to work effectively with client, families/carers, multi-disciplinary team members, other health professionals and agencies in order to meet service goals and enhance health outcomes.
4. Provide case management services, including liaison with patients and their families, other health professionals, supervisors, students, and researchers.
Leadership and management
5. Provides clinical practice supervision to less-experienced practitioners, work experience students or those involved in observational clinical placements; and provides direction to assistant and support staff.
6. Demonstrates ability to participate in quality or service improvement activities under the clinical practice and / or operational supervision of a more experienced practitioner.
