It is the location for Specialist Palliative Care Services in the ACT for which all patients must have a referral.Our palliative care services are tailored to each patient and are delivered by our team of Doctors, Nurses, Physiotherapists, Social Workers and Pastoral Care professionals. Our goal is to help patients achieve dignity, comfort and maximise the quality of life as they progress into end of life.About The Role:As a Palliative Care Nurse Practitioner, you will play a crucial role in assessing, planning, implementing, and evaluating care for patients with serious, life-limiting illnesses. Your responsibilities will include:Providing comprehensive symptom management, pain relief, and psychosocial support to patients and their families.Conducting thorough assessments and developing individualised care plans to improve the quality of life for patients.Offering education on advanced illness, treatment options, and end-of-life care tailored to each patient’s needs.Collaborating with an interdisciplinary team to ensure holistic, patient-centred care.Maintaining accurate documentation and advocating for compassionate, dignified care at all stages of illness.Qualifications:Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) AND endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia (mandatory).Proven experience as a Nurse Practitioner, with exceptional clinical skills and expertise.
