About the Care Coordinator –Transport Services position
The Care Coordinator - Community Transport is an important role in undertaking the assessment, planning and coordination of services and supports for older people, people living with a disability and people requiring short-term medical care to optimise their choices and provide the life-changing care and support they choose.
The incumbent will need to be caring and respectful and have a solid understanding of how your support and the support of your team enables individuals to live with independence and dignity.
About the Living Well Care Coordination (Community Transport) function
The Living Well Care Coordination (Community Transport) function provides expert assessment, planning and care coordination to individuals who require community transport to achieve their social, wellbeing, medical and community participation goals. Participants access the Community Transport Service through the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), Community Assistance and Temporary Support (CATS) and National Disability Insurance Scheme (NDIS). A person-centred, warm and individualised approach for each person is the essence of the CRCS Care Coordination function within the Living Well Model.
This is a part time (22.5 hours per week) ongoing position based at Trevor Pearcy House, Bruce. This role will be required to work from other locations such as Belconnen Community Centre or the Westfield CRCS Hub (or other location) as required.
* Recruitment, onboarding, supervision and performance development of Community Transport Drivers
* Undertaking person centred assessment, planning and coordination of services and supports for older people to enable them to optimise their choices and receive care and support they choose to enable them to live at home safely and independently
* Building effective and collaborative partnerships with participants and their advocates in addition to other colleagues and other key stakeholders
* Documentation and reporting requirements relevant to the role
* Follow work instructions, best practice guidelines and safe work guidelines
* Undertake quality audits, develop improvement plans and achievement against quality standards
* Feedback and complaints management and reporting
* Safety and risk management
Additional requirements for the role:
* First Aid and CPR Certification
* Certificate IV or Diploma qualification in Aged Care related field (or working towards or willing to obtain)
* CRCS employees must hold a satisfactory Working with Vulnerable People registration and have this with them at all times whilst employed by CRCS.
* CRCS employees are required to undertake a National Criminal History Clearance by CRCS prior to commencing and every three years thereafter as a condition of employment. Employees are required to advise CRCS of any convictions that may affect their eligibility to work or performing the inherent requirements of their position.
* Current Driver’s Licence and access to a registered, comprehensively insured motor vehicle for work related purposes.
