Take charge of clinical duties, design patient care plans, and build strong partnerships within a multidisciplinary team. Work alongside GPs, nurses, and external services to deliver high-quality primary care, driving better health outcomes for our community.Reporting to the Clinical Nurse Coordinator, this is a 0.6FTE fixed-term role until 30 June 2025.Your Passion and Commitment will see you:Deliver clinical care, including triage, immunisations, wound care, and chronic disease management.Coordinate care plans, health assessments, and health education programs.Work closely with a multidisciplinary team, ensuring holistic patient care.Provide person-centred care, collaborating with patients, families, and service providers.Respond effectively to changing situations and escalate issues as required.To succeed in this role you will have:AHPRA Registration as a Registered Nurse (Division 1) and Nurse Immuniser (highly desirable)A strong background in a General Practice setting with a solid understanding of its connection to the broader health sector and community health prioritiesOutstanding interpersonal, communication, and problem-solving skillsExtensive experience with recall and reminder systems, data, and clinical software systemsStrong organisational skills with the ability to prioritise tasks effectivelyFor further information about this role, please refer to the position description (available via the YourCH Careers Hub).YourCH has fostered an organisation-wide supportive and diverse team that allows staff to bring their whole self to work, both personally and professionally.
