About the position
Based in Ipswich, we are looking for an experienced professional who is able to hit the ground running. The Client Services Facilitator position is a dynamic role that acts as a key bridge for families and clients accessing our child and youth programs and supports, which include in-home support, community access, after school care, and vacation care. This role ensures the right information is up to date and at hand to ensure our fantastic support workers can deliver safe, quality care.

Key Accountabilities include:
1. Facilitate quality client service delivery for children and young people with a disability across Logan, Western Suburbs of Brisbane, and the wider Ipswich region.
2. Work with relevant stakeholders to understand individual client support needs and goals, including health and welfare requirements (e.g., seizure, medication, challenging behaviour supports).
3. Work with rosters and recruitment teams to ensure optimum support worker matching.
4. Ensure client files are up to date, responding quickly to provide support for situations as they arise.
5. Support program/activity design, planning, and resourcing with the Client Services Manager.
6. Where necessary, provide direct support to clients.
7. Prepare NDIS Progress Reports for plan reviews with the Intake and Quoting team.
8. Support and review incidents working with the Quality and Compliance team as required.
9. Participate in the Client Services Weekend On-Call Roster.
About you

To be eligible for this role, you would have at least 2 years' experience in client coordination, preferably in the disability sector and/or with young people, with a relevant qualification in community services or a willingness to work towards one.
1. Excellent administration skills and comfortable with using digital tools.
2. Good decision-making and communication skills ensuring effective supports and incident response.
3. Excellent attention to detail.
4. Valid Australian Driver’s Licence along with a comprehensively insured vehicle.
5. Current First Aid & CPR Certificate.
6. Blue Card & Yellow/NDIS Worker Screening Clearance Card.
7. Strong work ethic and good reliability.
This position is part-time (0.8 FTE) aligned with a Level 3 classification in the Social Community Home Care and Disability Services Award.
