About the Role:
Are you a Nurse Practitioner looking to make a real difference? Our client is seeking passionate professionals to join their Day and After-Hours Program, delivering vital clinical care to underserved communities across Tasmania. This mobile healthcare service provides accessible, no-cost healthcare to vulnerable populations, ensuring they receive the support they need.
As part of a committed team, you will work in partnership with Community Service Organisations to provide essential healthcare services in a flexible and supportive environment .
Key Responsibilities:
* Deliver high-quality clinical care within your scope of practice and AHPRA registration.
* Engage with patients in a respectful, non-judgmental, and culturally sensitive manner.
* Facilitate referrals to ensure ongoing patient support and follow-up care .
* Collaborate with Community Sector Organisation Partners and key stakeholders.
* Assist with patient behaviour management and de-escalation when required.
Qualifications & Experience:
* A passion for improving primary healthcare access in Tasmania.
* Relevant clinical qualifications and experience .
* Completed or working towards Nurse Practitioner Candidacy with authority to practice.
* Strong communication and documentation skills.
* Experience with Medical Director or Pracsoft (or willingness to learn).
* Full working rights in Australia .
* Commitment to supporting and advocating for people experiencing homelessness .

Mandatory Requirements:
* Current AHPRA registration .
* Professional indemnity insurance .
* Tasmanian Working with Vulnerable People Registration (or ability to obtain).
* National Police Certificate (issued within the last 12 months).
* Proof of COVID-19 vaccinations .
* Current Australian Driver's Licence (desirable).
