Home Support Aged Care Assessor - Single Assessment ServiceWe are seeking a dedicated Home Support Assessors to join our team on a locum basis. In this role, you will conduct comprehensive assessments for older Australians to identify their care needs and connect them with services that enable them to live independently and with a high quality of life.You will play a crucial part in supporting older individuals, promoting their independence, and ensuring they receive timely and quality assessments in alignment with the Department of Health's requirements.About the ClientThis role is based with a leading aged care and health service. The organisation's Assessment Services team is committed to helping older Australians maintain their independence and dignity by providing them with tailored, high-quality assessments. Their mission is to ensure that every assessment reflects the specific needs and aspirations of each client, contributing to better lives for older Australians.Key ResponsibilitiesUndertake a designated number of assessments each week for older adults to identify their needs and connect them with appropriate services.Complete assessments and provide detailed reports, ensuring they are finalised within one business day.Engage and build rapport with clients, their families, and carers to ensure their needs are clearly understood and met.Work closely with team members and managers to ensure the efficient scheduling and execution of assessments.Complete allocated Support Plan Reviews within 7 days and manage Reablement and Linking Support follow-ups in a timely manner.Ensure all assessments and services meet the Department of Health's guidelines and contractual requirements.Track and report assessments, mileage, and leave accurately using the company's systems and policies.Actively contribute to team meetings and departmental activities aimed at improving processes and service delivery.Selection CriteriaProven experience in conducting client assessments and/or service delivery in a health, community, or human services-related role.Strong communication skills (written and oral), with the ability to liaise effectively with clients, families, carers, and internal/external stakeholders.Excellent time management and organisational skills, with the ability to manage multiple assessments and tasks efficiently.Previous experience in aged care, health services, disability services, or community service coordination.Familiarity with the Commonwealth Home Support Programme (CHSP) and Home Care Packages.SOA2 is essential, other MAC learning modules desirableWhat's on OfferThe chance to make a real difference in the lives of older Australians by supporting their independence and improving their quality of life.A collaborative work environment with ongoing professional development and opportunities for learning.Great casual hourly rate plus generous travel allowance, and weekly pay
