Where you'll be working Northern Sydney Home Nursing Service, located in Building 8 Macquarie Hospital, North Ryde. Care is delivered to people in their homes across the NSLHD catchment area. What you'll be doing Provision of a range of social work services to patients of NSHNS including psychosocial assessment of clients; identification of issues such as child protection issues and elder abuse requiring mandatory reporting; counselling; advocacy and case management; and referral to appropriate services. A Bachelor or post graduate degree in Social Work and eligibility for the Australian Association of Social Workers (AASW). Experience at a Level 2 Social Work or equivalent level, and demonstrated skills in counselling/casework, with individuals and families, both short and long term. Ability to demonstrate advanced reasoning skills and operate autonomously with minimum direct clinical supervision. Demonstrated proficiency in ability to work autonomously with limited supervision. Have the ability to prioritise work, manage a demanding workload, apply high level reasoning skills and have independent professional judgement. Experience working in an acute hospital or community health setting; working with people with chronic disease, dealing with issues of loss, grief and bereavement; assessment, case management and discharge planning.
