Summary: As Clinical Care Coordinator, you will lead the clinical team to ensure and maintain excellent care whilst simultaneously improving the quality of life for our residents.
You will be instrumental in ensuring and upholding compliance standards as well as mentoring your team.Responsibilities: Supervise the delivery of care services, ensuring they meet safety, effectiveness, and regulatory standards.
Lead, mentor, and support the nursing team, including managing schedules, conducting performance reviews, and fostering a positive work environment.
Develop and review individualised care plans, collaborate with multidisciplinary teams, and address changes in resident needs promptly.
Commit yourself to continuous professional development.
Ensure accurate and timely documentation of care plans, incidents, and resident records.
Key Requirements: Prior experience as a Clinical Care Coordinator (or similar leadership position) is highly preferable, but not essential.
Flexibility to work weekends (if needed).
Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) Excellent clinical knowledge, leadership, organisational, and communicative skills with the ability to lead, manage and motivate a clinical team effectively.
Strong knowledge and experience of aged care accreditation, regulations, standards, and best practices.
