We are seeking an experienced Care Manager to join our team in Tenterfield, NSW, responsible for ensuring residents receive safe, high-quality care in an aged care setting. This key role involves leading and managing the care team, ensuring compliance with care governance, and overseeing the well-being and satisfaction of residents.
Key Responsibilities:
* Care Governance: Ensure that care practices adhere to regulatory standards and best practices.
* Resident Satisfaction & Experience: Lead initiatives to improve the quality of life for residents, ensuring they receive person-centred care.
* Care Team Leadership & Management: Provide direction and support to the care team, ensuring effective and efficient operations.
* Care Management: Oversee the care plans and ensure residents' needs are met in a timely and compassionate manner.
* Resident Classification & Subsidy: Ensure proper classification of residents for appropriate funding and subsidies.
* Quality & Safety: Ensure compliance with safety protocols and maintain a focus on quality care.
* Reporting & Accountability: Maintain accurate and timely documentation and reporting of care activities.
* Monitoring & Compliance:
o Oversee resident admissions, ensuring they meet quality and compliance standards.
o Evaluate and monitor care quality, addressing any gaps or deficiencies.
o Investigate care incidents and address concerns promptly.
o Track clinical indicators, physical restraint, psychotropic medication usage, and antimicrobial stewardship to ensure the best outcomes for residents.
Experience & Qualifications:
* Minimum of five (5) years of experience as a clinical nurse, care coordinator, or care manager, with at least one year in an aged care setting.
* Proven ability to lead teams and manage complex care scenarios in an aged care environment.
* Strong knowledge of regulatory requirements, care governance, and clinical standards in aged care.
