What you'll be doing
The Clinical Nurse Consultant provides expert clinical advice to patients/clients, carers and other health care professionals with complex healthcare needs within chronic kidney disease speciality.The Clinical Nurse Consultant is a clinical expert who, through leadership, facilitates excellence in research based clinical practice, education, research and professional leadership. The Clinical Nurse Consultant models the CORE (Collaboration, Openness, Respect and Empowerment) values of the organisation.
The overall purpose of this role is to identify, initiate and collaborate in the development and provision of programs to improve the early detection and management of people at high risk of developing chronic kidney disease in the Illawarra Shoalhaven Local Health District.
Role Specific Responsibilities
1. Provide evidenced based, client-centred education and screening to people and groups at high risk of chronic kidney disease.
2. Initiate and coordinate health promotional activities for patients, with modifiable risk factors, to delay progression of their Chronic Kidney Disease.
3. Provides pre dialysis education for patients approaching End Stage Kidney Failure
4. Establish and maintain key networks, partnerships and collaborations with agencies that manage people who are at high risk of chronic kidney disease.
5. Provide evidenced based education to health professional staff in the Illawarra and Shoalhaven region that cares for people at high risk of chronic kidney disease.
