In this diverse and rewarding role, you will work collaboratively within a multidisciplinary team across clinic and community settings, including CALHN's acute and sub-acute sites. Your focus will be on delivering culturally appropriate clinical interventions and assessments that support hospital avoidance, substitution, and reduced length of stay in the acute sector. You will play an integral role across services such as Hospital in the Home, Refugee Health Service, O'Brien Street Practice, and the Hospital Avoidance Supported Discharge Service, including its Outreach Team.This position offers a unique opportunity to contribute to the health and wellbeing of vulnerable clients with complex needs through coordinated and integrated service responses.
