This role works toward providing young people (aged 15+9mths to 21) skills and networks to be able to transition into adulthood successfully from the out-of-home care setting:
* Case manage a number of young people who are transitioning from care.
* Undertake assertive outreach and support that includes varying levels of intensity.
* Motivate, encourage, and empower clients to address areas of personal difficulty.
* Actively support, encourage, and maintain the development of positive relationships, networks, and linkages within the young person's community.
* Actively promote the establishment and maintenance of positive family relationships.

THE IDEAL CANDIDATE – what you will bring.
* Working knowledge of the Child protection system and the complexities of young people transitioning from care.
* A demonstrated understanding of the social and political issues impacting young people.
* Demonstrated experience and knowledge of case management framework and practices.
* Valid Victorian Driver’s Licence | Working With Children Check | Police Check.
* Minimum requirement of a Diploma of Community Services or other relevant tertiary qualifications and/or experience appropriate to the role.
