Job Description:
We seek a self-motivated, independent individual to develop individualised case management plans for individuals using a case management framework to identify issues, assess health, cultural and educational needs and propose suitable interventions. In addition, he/she will be required to provide referrals to case management services to achieve better outcomes for young people. Liaising with Youth Justice and QPS to assess the risks and needs of young people engaging in anti-social behaviour and coming to police attention will be one of the key responsibilities.
Candidates Need:
1. Diploma level qualification in Social Work, Child, Youth and Family Intervention or evidence in the course is also considered appropriate.
2. An extensive understanding of Aboriginal and Torres Strait Islander peoples, cultures and protocols and the issues which impact upon them in contemporary society.
3. Demonstrated ability to provide appropriate information on a range of services and referral pathways available for young people and families specific to their circumstances, including accommodation, counselling, family support, legal, education, employment, drugs and alcohol.
4. Demonstrated case work experience.
5. Experience in collaborative program arrangements with relevant agencies, other service providers, and key stakeholders.
6. Sound understanding of the role of a Case worker.
7. Willingness to participate in training for the role.
8. Effective oral and written communication skills.
9. Computer-literate and the ability to complete reports.
10. Sound organisational and time management skills.
11. Current Driver’s Licence.
12. Blue Card.
