Support Coordinator/Recovery Coach Role

Role Overview:
Support Coordination: Assist individuals with psychosocial disability to participate more fully in their community by:
* Strengthening their ability to coordinate supports within National Disability Insurance Scheme (NDIS) individualised funding packages
* Supporting engagement with mainstream services and informal networks
* Developing capacity and resilience in their social network.
Recovery Coaching:
* Develop a recovery-enabling relationship with clients, including family and carers
* Support clients to develop and implement individual recovery plans
* Coach clients to increase recovery skills and personal capacity, including motivation, strengths, resilience and decision-making
* Collaborate with broader systems, including available services and resources
* Support clients to become active participants in the NDIS
Requirements:
* Cert IV in a Community Services related discipline or higher qualification is required
* Experience in human services, particularly understanding service delivery for clients with persistent and complex mental health needs
* Ability to meet deadlines in a high demand work environment and exercise judgment and initiative
* Excellent organisational and time management skills to hold a caseload of up to 25 participants
* Ability to work independently and within a team
* Must be eligible to apply/currently hold a WWCC
* Must hold/be willing to apply for NDIS worker screening clearance
* Current Driver's License is required
