The role
As Team Leader – Triage and Delegate Services, you will enjoy leading and overseeing a team of professionals in the execution of the single assessment process for aged care customers. You will ensure that assessments are conducted efficiently, accurately and in compliance with regulatory and organisational standards.
This is a newly created opportunity in support of Suncare’s delivery of the Single Assessment System for aged care, and offered as permanent, full-time opportunity (Monday to Friday). Given our number of Suncare offices, including a head office in Maroochydore, there is flexibility with your office location.
More about the role
As an accountable party in providing services under the Aged Care Act, you will be mindful and act with due diligence in the discharge of your duties. Reporting to the Operations Manager – Assessment and Clinical Services, these duties will include:
* Managing the day-to-day operations of the triage and delegation teams, ensuring smooth and compliant coordination of assessments within contractual timelines.
* Monitoring team performance through key metrics, such as assessment allocation rates, data accuracy and customer feedback.
* Overseeing the accurate and timely input of assessment data into relevant systems ensuring consistency and compliance with organisational standards.
* Ensuring assessments are conducted in accordance with best practice guidelines, regulatory requirements and the Support at Home model.
* Serving as a primary point of contact for escalated issues related to assessments, resolving concerns from customers, delegates and external stakeholders promptly.
* Contributing to the development and adherence of policies, procedures and framework and undertaking regular review of quality management processes.
* Actively supporting and participating in clinical supervision, team meetings, and individual catchups and provide guidance and direction to team members to ensure effective performance, safety, and compliance with standards.

What you will bring to our team
In addition to leadership experience and customer service ethos, you will possess a positive attitude for working in the aged care industry. You will also bring to our team:
* Tertiary qualification in a health-related discipline.
* Current unrestricted AHPRA registration or part of, or eligible to be part of, a relevant professional association.
* Good knowledge and understanding of the Aged Care Act.
* Demonstrated leadership skills and practices for the effective leadership of a multi-disciplinary team.
* Demonstrated ability to apply continuous quality improvement principles in service development.
* High level customer service skills and ability to form positive relationships with community members and stakeholders.
* Advantageous is current experience as a clinical Aged Care Assessor.
