An exciting opportunity is now available for a Full-time Care Coordinator to join the RivMed Integrated Team Care Unit. The successful applicant will join an established, growing team.
This is an Identified role and is only open to Aboriginal or Torres Strait Islander Applicants only.
The Role
The position will sit within the RivMed Integrated Team Care Unit, situated in our Medical Offices in Edward Street, Wagga Wagga.
The Care Coordinator is required to establish links with local Aboriginal and Torres Strait Islander communities to promote the importance of improving health outcomes and encourage and support the increased use of health services. The coordinator achieves this through individual consultations in the clinic and through outreach work.
Key Responsibilities
1. Establish links with local Aboriginal and Torres Strait Islander communities to promote the importance of improving health outcomes and encourage and support the increased use of health services.
2. Provide care-coordination and care planning services including engagement with patients and carers and case conferencing with GPs and other health professionals.
3. Manage all aspects of care coordination from initial assessment and active care-coordination, through to managing clients' exit from the program.
4. Coordinate care with, and between, diverse service providers including social and health care services as well as family and carers where appropriate and consented to by the client.
5. Support clients who smoke to cease smoking.
6. Assess and validate referrals for against service eligibility criteria.
7. Provide services in a culturally sensitive manner.
8. Record all client contacts in RivMed's patient information system.
9. Contribute to the effective operation of the team by supporting colleagues through clear and effective communication, sharing of appropriate and relevant information and working cooperatively with others.

Selection criteria
1. Aboriginality - This is an identified Aboriginal or Torres Strait Islander position and candidates will be required to provide confirmation of Aboriginality.
2. A demonstrated understanding of the issues and barriers that impact upon the health outcomes of Aboriginal people.
3. Minimum Certificate IV level qualifications relevant to Aboriginal Primary Health Care work e.g. Aboriginal Health Practitioner / Worker, Enrolled Nurse, and Registered Nurse.
4. Experience in delivering health services for people living with chronic disease and previous experience in care coordination.
5. Knowledge of primary health and the health care sector.
6. Experience / ability to liaise effectively with General Practitioners, nurses, specialists and allied health professionals.
7. Knowledge and understanding of local Aboriginal and Torres Strait Islander communities and cultures.
8. High level communication skills (verbal and written) as well as the ability to write case notes.
9. All employees will be required to have a Police Check and or Working with Children Check in accordance with legislative requirements or as determined by the Board.
10. Applicants must hold a current driver's licence with an ability and willingness to travel throughout the Murrumbidgee area. (RivMed vehicles provided)
