We work across acute, sub-acute and community settings, including the Royal Hobart Hospital, the largest teaching Hospital in Tasmania.This role is based in the Community Transition Care Program which provides short term care (up to 12 weeks), delivered at home, to optimise the functioning and independence of older people after a hospital stay. The team consists of nursing, allied health and case managers with brokered support services. As an Occupational Therapist, you will deliver high quality, person-centred services and work closely with the other members of the team, including other Occupational Therapists and Allied Health Assistants. You will be supported by Senior Occupational Therapists, the broader CTCP team and will have access to regular professional development opportunities.Please note this is a non-rotational position.
