QualificationsCurrent registration as a Registered Nurse (RN) with AHPRA.Proven experience in clinical care management.Strong communication and interpersonal skills.Ability to work independently and as part of a team.Passion for providing outstanding care to clients.Preferred SkillsClinical Expertise: Proven experience in providing direct client care and ensuring safety.Government Funding: Good understanding of the government funding environment for aged care and NDIS (not compulsory but preferable).Hospital Discharge: Good understanding of the hospital discharge environment.Health Assessments: Good understanding of health assessments and developing care plans to meet clients' care needs.Multitasking and Time Management: Ability to multitask and manage time effectively.Communication: Excellent verbal and written communication skills.Computer Skills: Intermediate to advanced computer skills, including familiarity with CRM systems. #J-18808-Ljbffr
