The Role

This role will see you engage with those experiencing homelessness and provide case management to assist with overcoming barriers and successfully maintaining an independent tenancy:
Key responsibilities:
* Undertake networking opportunities within the community to successfully engage with clients experiencing homelessness
* Coordinate services and provide supported referrals for clients for housing, health care, employment, financial and other relevant services
* Advocate on behalf of clients accessing other services
* Develop and implement support plans with clients
* Ensure effective and appropriate communication with staff within all programs in the delivery of the service via regular team meetings
* Ensure a diligent approach is taken to record keeping and the principles of confidentiality and privacy
* Provide regular communication with the Housing Services Coordinator and assisting with relevant data collection and reporting
This position is full time, fixed term based at our Maryborough office.
About you:
You will demonstrate experience in a similar role, including experience in case management and/or provision of support services. In addition, you will have relevant qualifications and/or experience in a human services framework for people presenting as homeless, including a demonstrated understanding of the needs of people experiencing homelessness or housing crisis and knowledge of relevant local support services. You will also have:
* A talent for building relationships and working in partnership with a diverse range of stakeholders
* Excellent communication skills, including the ability to communicate clearly within complex and diverse conversational contexts
* A client-centered approach, including provision of respectful, supportive, and confidential customer service
* Self-motivated with the ability to work independently as part of a wider team and adapt in a fast-paced environment
* The ability to achieve work objectives, even in difficult circumstances whilst remaining positive and calm
* A current drivers licence
* A commitment to the right of all people to affordable and sustainable housing
Please note that a satisfactory police check is required for all staff and those with client related responsibilities are required to provide a NDIS and Working with Children Check (WWCC).
