Why this role matters
The Salvation Army Australia’s Homelessness Stream provides high-quality, person-centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services.
About the role
We are seeking a values-driven and proactive Housing Case Manager to advance the issue of homelessness within the Wimmera South West and to provide assessment, information and referral, support, case management, counselling for clients who are socially isolated, homeless or at risk of homelessness.
Reporting to the Team Leader, this is a fixed-term (until 2026), part-time (30.4 hours per week) position based in Horsham,Victoria.Salary and conditions are in accordance with the SCHADS Award Level 5.
You will successfully
* Coordinate supports through case management practices, and referral to key support agencies.
* Develop appropriate and realistic housing plan for all clients participating in the program to assist clients in making informed housing decisions.
* Provide an environment and activities that enhance client’s ability and well being to enable them to work towards their goals in a supportive and structured manner.
* Maintain knowledge of and participate in relevant risk prevention.
* Provide reports (oral and written) to the Team Leader or Manager as requested.
You will have
* A social work qualification or other relevant social tertiary qualifications and/or substantial experience.
* A current and valid Victorian Employee Working with Children Check.
* Current VIC Driver licence.
