Clinical Care Coordinator Job
As a Clinical Care Coordinator, you will lead the clinical team to ensure excellent care and improve the quality of life for our residents. You will be instrumental in ensuring compliance standards are upheld and mentoring your team.
Main Responsibilities:
* Supervise care services delivery to meet safety, effectiveness, and regulatory standards.
* Lead, mentor, and support the nursing team by managing schedules, conducting performance reviews, and fostering a positive work environment.
* Develop individualised care plans, collaborate with multidisciplinary teams, and address resident needs promptly.
* Participate in quality assurance activities, conduct audits, and implement improvements to maintain high care standards.
* Ensure accurate documentation of care plans, incidents, and resident records.
Key Requirements:
* Prior experience as a Clinical Care Coordinator or similar leadership position is highly desirable but not essential.
* Willingness to work weekends.
* Registration with the Australian Health Practitioner Regulation Agency (AHPRA) is mandatory.
* Excellent clinical knowledge, leadership, organisational, and communication skills are required.
* Strong knowledge of aged care accreditation, regulations, standards, and best practices is necessary.
