Community Care TASMANIA have a 3 month fixed term position for a Brokerage Service Coordinator. Ideally this will be an opportunity for the right person to drive the brokerage program with the potential to make it a permanent role.Summary:The Brokerage Service Coordinator is responsible for the management and delivery of support and services to brokered clients for CCT.Primary Tasks:
•Daily management of the Brokerage email account.
•Ensuring appropriate services (e.g., personal care, domestic assistance) are arranged for clients through external providers.
•Liaising with external providers to schedule and confirm service delivery.
•Creation of client profiles in Client Management System and creating client individual care plans.
•Managing changes or cancellations of services as required.
•Assessing Support Worker/ regional capacity and accepting/ declining services accordingly.
•Acting as the primary point of contact between the organisation, clients, and service providers.
•Building and maintaining strong relationships with external providers to ensure quality and compliance & growth.
•Ensuring all documentation related to services is accurate and up-to-date, and filed correctly.
•Monitoring service delivery to ensure compliance with organisational standards, contracts, and aged care regulations.
•Managing contracts and agreements with external providers.
•Addressing service delivery disruptions or client dissatisfaction.
•Ensuring all incidents are documented accurately and promptly in line with organisational policies and aged care regulatory requirements.
•Follow non response procedures
About you:We are looking for an organised and enthusiastic individual who wants to make a difference and holds the qualifications and skills enabling them to perform the primary tasks listed above and:
•Experience, knowledge and understanding of culturally and linguistically diverse (CALD) issues in relation to frail elderly clients, people with disabilities, people with dementia, and implications for clients, primary carers, families and community agencies.
•Demonstrated experience of well-developed skills in managing time, setting priorities, planning and organising your own work.
•Demonstrated awareness and understanding of issues confronting people with disabilities in their home or a community setting.
•Demonstrated ability to exercise a high level of interpersonal skills, with the ability to liaise effectively with clients, support workers and a range of external stakeholders.
•High level of computer skills including the use of Microsoft programs such as Word and Excel, and the ability to readily adapt to the use of software programs.
•Experience and demonstrated ability to manage and implement the delivery of government funded programs.
And holds or has the ability to apply for the follow:

Current WWVP with NDIS endorsement
NDIS Workers Orientation Module.
