This is an exciting opportunity for a motivated social worker to join the Social Work Department at St George Hospital.
You will be part of a large, dedicated and highly skilled department working with culturally and socially diverse patients and their families.
This permanent part time level 1 / 2 position is within the Cancer and Chronic Care team, working with patients and families who are on Haemodialysis.
There will be inpatient and outpatient connect with patients and families, the position facilitates the assessment, counselling, and case management of individuals experiencing chronic health vulnerabilities.
All Social Work positions are required to provide support to other areas within the hospital as needed.
Applicants are advised that at times the position involves working with vulnerable people and people experiencing trauma.
Applicants are also advised that this position includes ongoing participation in the after-hours on call crisis service, including weekends.

Minimum Requirements: A Degree in Social Work from a recognised University and eligible for AASW membership.Current unrestricted Class C NSW driver's license.Post Graduate Social Work clinical experience demonstrating competencies in comprehensive assessment and case management of frail older clients with complex health problems and other community support needs.Demonstrated knowledge of guardianship, financial management and elder abuse issues and related legislation.Demonstrated comprehensive knowledge of community aged care resources.Demonstrated ability to work independently and as part of a multidisciplinary team.Demonstrated organisational skills and ability to manage a demanding caseload.Demonstrated high level communication, report writing skills and computer skills.
