About the role: As a CCM on the TRANSPAC portfolio, your primary responsibility will be to quickly develop a goal-orientated care plan for each customer on the program.Within short-time frames, you will liaise with the hospital to confirm the customer's discharge plan, develop personalised care plans, and manage the urgent deadlines until the end of their program involvement.Collaboration and coordination with internal stakeholders such as nurses and Allied Health professionals will be crucial to delivering comprehensive care.Whilst encompassing large components of a standard CCM role, you will also ensure the highest quality services are delivered on-time and against key TRANSPAC performance measures.This role is a full-time permanent position and will involve regular travel around the Ryde, North Sydney and Northern Beaches areas of Sydney.About You: Previous experience in community-based case management is preferred.Understanding of the TRANSPAC program would be advantageous.Current driver's licence and a reliable vehicle with insurance.Strong communication skills, both verbal and written.Demonstrated empathy and working to strict deadlines.Excellent problem-solving abilities and critical thinking skills.Good organisational and time management skills.Cultural competence and the ability to work with diverse populations.Collaboration and teamwork skills to work effectively with interdisciplinary teams.Internal applications close: 5pm, Friday 1st March 2024.Please review carefully and, if you are interested in applying, please seek endorsement from your current manager first before submitting your updated CV via the links above (no email applications please).A position description and salary bandings will be discussed later in the recruitment process.If you have any questions, please email us at ******
