About the role We are currently looking for a team of reliable and dedicated Support Workers to support our clients in the Grafton. We want to provide a safe and loving environment. We are looking for a range of youth workers with a background in first aid mental health, and/or disability support. As an integral part of our team, you'll be encouraged to utilise your passion and skills to support our clients to grow, develop, heal and feel empowered. Skills Required : High level of administrative skills. Experience in suicide prevention. Mental health First Aid certification. Essential requirements: NDIS Worker Screening Check Employee - Working with Children’s Check Willingness to undergo pre-employment checks A valid Australian driver’s license with a reliable and comprehensively insured car A current First Aid and CPR Certificate A current certification in Trauma Informed Care (desirable) Youth Mental Health First Aid (Highly desirable) Responsibilities Document Management: All required documents must be added to the NH internal system. Ensure compliance with all documentation requirements Referrals: Handle referrals from Primary Health Networks (PHNs). Respond to referrals within 24 hours. Review and act on discharge summaries received. Make contact with clients based on discharge summaries. Client Support: Conduct risk assessments and develop safety plans. Provide weekly support, including working with risk assessments and safety planning. Link clients with appropriate services.
