About your role: Provide effective, efficient, competent and culturally appropriate client focused clinical care to CHSP clients as per current CHSP Program Manual 2020-2022. Provide leadership to effectively plan, manage and evaluate patient care at an advanced level, on all eligible CHSP clients within the CHSP Service and develop and maintain professional and multi-disciplinary networks within the CHHHS CHSP stakeholders resulting in efficient CHSP service delivery. Promote cooperation, teamwork and understanding in undertaking generalist and/or specialist nursing practices for effective health outcomes.
