As we continue to grow, a new opportunity for a Change Specialist is now available. You will play an integral part in change of the model of care across Residential Aged Care and Home & Communities. Reporting to the Program Manager, Model of Care, you will partner with members of the Inspirit Team and stakeholders to implement and embed change management initiatives to support successful implementation of Catholic Healthcare’s new Model of Care and inspire confidence in others to embrace positive change.
Championing the Model of Care change management process across the organisation, with a key focus on relationship-centred care will allow you to utilise your exceptional communication and interpersonal skills together with your passion for change and growth, and your belief that there is a better way to support aging.
Your ability to thrive in a fast-paced, dynamic environment and manage multiple priorities simultaneously, with a strong commitment to delivering high-quality results and driving positive change in aged care will aid your success.
Your role will include;
* Developing and implementing a change management plans, utilising best practice change methodologies and processes, to implement the Model of Care on time, in scope and in budget
* Evaluating and analysing effectiveness of change management plans (KPI’s)
* Designing and implementing training programs to effectively educate key stakeholders on the Model of Care
* Developing and executing communication plans that support the delivery of change management initiatives. •
* Ensuring that change initiatives meet established quality standards
* Assess complex situations, analyse data, and develop evidence based solutions
* Travel to sites across Sydney Metro and Regional NSW
The successful candidate will be highly organised, with an agile approach and the ability to influence and engage effectively with stakeholders, in addition to the following:
* Proven experience in change management with a focus on implementing transformative initiatives within a complex project or models of care
* Previous work history in the health or aged care sector preferred
* Strong understanding of aged care delivery models in both Residential Aged Care and Home and Community services, ACQSC standards, and knowledge of emerging trends and best practice in models of care
* Analytical mindset with exceptional problem solving skills
* Project management experience and familiarity with project management tools and methodologies
* Excellent time-management and prioritization skills
* Proficient MS Office and MS Project skills
