About Central Gippsland Health Central Gippsland Health (CGH) is a sub-regional health service and the major provider of health and aged care services in the Wellington Shire. CGH is located in the City of Sale which is 215 km east of Melbourne. The City of Sale is an ideal access point for the magnificent Gippsland Lakes, 90 Mile Beach and the Victorian Alps. CGH provides acute care as part of an integrated service delivery model across Wellington Shire and beyond. The service provides beds across three campuses (Sale, Maffra and Heyfield). Acute services include a full-time emergency department, critical care unit, neonatal critical care unit, three operating theatres, day procedure unit, oncology and dialysis services in addition to general medical services and sub-acute services including rehabilitation. CGH is also a leading teaching hospital, affiliated with Monash University's School of Rural Health and provides placements and clinical experience for students. About Department The Home Care Package Team at Central Gippsland Health provides support and package coordination to clients across the whole of the Wellington Shire. This includes administrative support, as well as in-home assessments and follow-up. About the Role As a Home Care Package Care Coordinator, you will be responsible for the implementation and coordination of services that support our clients to live safely and independently. Coordination of a client's package includes: Development of individualised care plans to meet the client's needs. Individual budget management ensuring the effective use of package funds. Maintain strong relationships with clients, families, and support services both internal and external to CGH. Qualifications, Skills and Experience Certificate III qualifications in community health related discipline and/or care coordination or equivalent training. High level communication skills at both an individual and group level with an ability to establish rapport across a diverse range of people.
