About the role
An exciting opportunity is available to be a part of Lakeside Living Edenhope Residential aged care home in the new Meaningful Life Model of Care.
The Meaningful Life Partner is responsible for the implementation of meaningful engagement that gives daily purpose to our individual residents, bringing joy to all that we care for. This will be delivered through meaningful activities and purposeful engagement that supports a holistic wellbeing.
Lakeside Living Nursing Home has been designed to accommodate 40 residents, ensuring quality and comfortable accommodation and care in a friendly home-like environment with a seven day a week activity program.
Key Responsibilities
* Implement meaningful engagement, across the care community, that include activities that reflect a person-centred model of care, in line with the Aged Care Quality Standards.
* To deliver meaningful activities and person-centred engagement, that meets residents’ goals, needs and preferences.
* Be responsible with the direction of the Meaningful Life Coordinator, to complete, all meaningful life residents’ assessments, care plans, resident iCare profiles and documentation.
* Provide support to other team members, volunteers and students in the care community.
* Ability to manage time effectively whilst following daily schedule, including the setting-up and cleaning up after activities.
* Attend regular meaningful life team meetings and care community meetings as directed by the coordinator.
* Advise coordinator and assist with organising resources required to support the meaningful life program.
Skills and experience
* Hold a Certificate III in Aged Care (or equivalent) or Certificate IV in Leisure and Health (or equivalent) or willingness to obtain.
* A Qualification in dementia care is desirable.
* Demonstrate an understanding of Aged Care Quality Standards.
* Excellent communication skill with the ability to work as part of an inter-disciplinary team, with strong computer and customer service skills.
* Experience with the Aged Care software program iCare is desirable
* Victorian Driver’s Licence
