The Clinical Nurse Community Health contributes to the provision of high-level community nursing support through care coordination, planning and support for clients with multiple co-morbidities and their carers across the continuum of care. The Community Health Nurse practices autonomously and within a multidisciplinary team to provide nursing services within a rural health service in a community setting.The key purpose of this role is to undertake a variety of general nursing duties to ensure appropriate care for clients in accordance with the SWHHS core values and strategic plan and deliver a collaborative, holistic and quality service tailored toward the individual client's needs.The very nature of what we do and who we are is the real opportunity.
